Bill and collect payment; submit claims to insurance plans for vaccine or preventive care when applicable.

Improve quality, prevent fraud, confirm identity, and ensure prescription validity across states.
Public Health & Safety
Report vaccine data to registries if required by law or with your consent; help prevent or control disease.
As Required by Law
Comply with federal, state, or local laws and lawful government requests.
3. Your Rights
- Access – Request copies of your records and prescriptions.
- Amend – Ask us to correct inaccurate information.
- Accounting – Request a list of certain disclosures.
- Restrictions – Request limits on uses/disclosures (we may not always agree).
- Confidential Communications – Request we contact you in specific ways.
- Paper/Electronic Copy – Request this Notice in paper or electronic form.
- Complain Without Retaliation – File complaints with us or HHS.
4. How We Protect Your Information
- Prescription PDFs are stored only on your device; we do not retain copies.
- We use encryption and HIPAA-compliant processes for any transmissions.
- Optional data (insurance card, ID scans, registry results) is used only for care coordination and payment.
5. Special Features in QwikCare
- CareMap: Optional insurance info enables local pricing transparency.
- Registry Lookups: With consent, we can verify and compile your vaccine history.
- Apple Health: You may elect to sync vaccine records to Apple Health on your device.
6. Changes to This Notice
We may update this Notice. The revised version will apply to all PHI we maintain and will be available in the App.
