1. Purpose of Agreement
This User Agreement governs your use of the Lifetime COVID Vaccine Prescription in-app purchases (“IAPs”) within the QwikCare app. By purchasing and using these products, you agree to the terms below. The Agreement establishes your entitlement to request COVID vaccine prescriptions for life through encounters with licensed providers affiliated with Lemur PBC, subject to clinical review and standards of care.
2. In-App Purchases (IAPs)
Lifetime COVID Vaccine Prescription
- Permanent personal right to request COVID vaccine prescriptions in any U.S. state for life.
- Tied to your Apple ID; restorable across Apple devices.
- Establishes you as Head of Household, enabling purchases for others.
Family Lifetime Prescriptions
- Purchasable by a Head of Household for children/dependents under 18 with legal authority.
- Transfer to the dependent’s device and Apple ID at age 18 via AirDrop, text, or email.
Friends & Loved Ones Lifetime Prescriptions
- Purchasable by a Head of Household for any adult (spouse, friend, loved one).
- Recipient redeems via link; entitlement permanently attaches to their Apple ID.
3. Data Use, Privacy, and Consent
By purchasing any lifetime prescription IAP, you consent to Lemur PBC using your legal name, date of birth, phone number, and location to prevent fraud, ensure prescription validity, and maintain continuity of care across name changes and state lines; filing claims with present/future insurers when coverage applies; verifying vaccine history via state/federal registries; optionally scanning your insurance card for accounting or price transparency in CareMap; and optionally scanning government ID to aggregate vaccine records. Lemur PBC complies with HIPAA and shares information only as needed for care, claims, or pharmacy verification.
4. Prescription Encounters
- First-encounter prescriptions are cash-pay encounters with Corey Amman, M.D., or another licensed provider.
- Providers may approve/sign immediately, ask follow-up questions, refer to an in-state provider, or contact you by text/phone if needed.
- Prescriptions are valid for six (6) months. You may request a new prescription anytime (e.g., travel, expiration, state licensing).
5. Prescription Documents
- Approved prescriptions are delivered as signed PDF files stored on your device.
- Lemur PBC does not retain a copy.
- You may print or share your PDF with your pharmacy using standard device tools.
- Prescriptions may include substitution language consistent with medical standards.
- Pharmacists verify patient identity and may contact Lemur PBC directly.
6. Insurance and Cost Transparency
- The lifetime IAP guarantees provider encounters and, when appropriate, prescriptions; it does not cover vaccine administration costs.
- Vaccine costs may be billed to insurance or paid out-of-pocket at pharmacies.
- If you scan your insurance card, CareMap may display local pricing information based on your coverage.
7. Apple Health Integration
- You may import records into Apple Health (self-reported administration, prescription records, registry history with consent).
- Records remain on your device unless you choose to share them.
8. Management of Family & Friends Prescriptions
- Heads of Household can view family entitlements for minors, gifted entitlements for adults, and redemption status (active, pending, refunded).
- Unredeemed lifetime prescriptions may be refunded or reassigned; once redeemed, they are tied to the person for whom the lifetime right was purchased, linked through the purchaser's Apple ID and gifting history.
9. Liability
- Lemur PBC guarantees access to licensed encounters and prescriptions when medically appropriate.
- Lemur PBC is not responsible for pharmacy stock, vaccine administration, or insurer reimbursement.
- Prescriptions remain subject to provider judgment, medical appropriateness, and applicable law.

For entitlement recovery, name changes, or technical issues: prescriptions@lemurpbc.com. For urgent medical concerns, contact your provider or dial 911.
11. Governing Law
This Agreement is governed by U.S. law and the laws of the State of North Carolina.
Acceptance
- You consent to data use for fraud prevention, vaccine verification, and insurance claims as described.
- You understand prescriptions last six months and require renewal.
- You are purchasing lifetime prescription rights, not vaccines.
- You agree to Lemur PBC’s standards of care and HIPAA-compliant handling of your health data.
